Consent and Declaration
The information on this application (“Information”) is collected by the provincial/regional body for the purposes of considering the Member’s application for approval as a Mentor (the “Purpose”). The Information shall be used by the provincial/regional body only for the Purpose and shall not be disclosed to any person, other than employees of the provincial/regional body, unless such disclosure is necessary to fulfill the Purpose. By submitting this application, the Member acknowledges that the provincial/regional body will, as well, use any information in its possession or control regarding the Member, whether collected from the Member or otherwise, for the Purpose and the Member consents to such collection, use and disclosure of the Information.
In addition, the provincial/regional body is responsible for determining if approved Mentors continue to meet the eligibility criteria and justify continued approval.
Declaration
I undertake, acknowledge and agree that,
1. I am a Member in good standing with a provincial/regional body or another Provincial accounting body (except in Alberta, where the mentor must be a member in Alberta);
2. the information provided in my application for approval as a Mentor is complete and accurate;
3. any false or misleading statement contained in this application may result in my application to become a Mentor being declined, and may result in this application being used in any proceeding by their provincial/regional body;
4. the provincial/regional body will collect, use and disclose information regarding me as required to fulfill the Purpose;
5. no findings of professional misconduct or conduct have been made against me by my provincial/regional, any professional accounting or regulatory body or any court and, to my knowledge, the provincial/regional, any professional accounting or regulatory body is not conducting any investigations into my conduct, there are no outstanding complaints of harassment or discrimination against me and there are no outstanding proceedings against me before the provincial/regional, any professional accounting or regulatory body with respect to my conduct;
6. I have not been found guilty of any criminal or similar offence and I have not been adjudged bankrupt or made an arrangement with creditors pursuant to the Bankruptcy and Insolvency Act; and
7. I am in full compliance with all the provincial/regional requirements, including requirements set out in the by-laws and regulations of the provincial/regional body, and the Rules of Professional Conduct;
8. I will abide by the administrative requirements and responsibilities of a Mentor as set out in the mandatory CPA Mentor orientation video and the fact sheet.
9. I will notify the provincial/regional body immediately if I become aware of an investigation or proceeding into my conduct, or a complaint of harassment or discrimination, being commenced by a provincial/regional, any professional accounting or regulatory body.
10. As a CPA Mentor, I cannot be a subordinate to the CPA Mentee.
11. While an employer of a CPA student/candidate can be an audit client of a CPA Mentor’s employer, as a CPA Mentor, I cannot be a part of the team assigned to the client.
I make this solemn declaration conscientiously believing it to be true and knowing that it is of the same force and effect as if made under oath.
